If I choose to discontinue my child's enrollment, I understand that I must cancel my plan by contacting the Mr. MusicMan Company of Chicago, and that my child's enrollment will end at the end of the last paid month.*

I consent to Mr. MusicMan using any pictures or video, that may include my child, for marketing and social media purposes.*
